At each New Eligibility or Re-evaluation meeting, the Eligibility Team (including parent/guardian) will consider:
1. the presence of an impairment,
2. adverse impact on educational performance, and
3. the need for specially designed instruction
The team must document their deliberations, including information about the review of data, the specific disability criteria, the exclusionary factors, and any recommendations in the meeting summary. Eligibility worksheets for each of the 14 disability classifications are used to help guide the team.
