Step 5: Upload your Proof of Identity

Use the secure document upload area below to submit a proof of identity for both you and your child

Uploading Directions: Follow the same steps to upload you used when uploading the registration form on this and subsequent pages. The fields with your name, email address, and school(s) should already be filled out. Be sure to click “Submit” after you have uploaded the document(s) and check that the confirmation of upload is displayed in the upload window.

Proof of Identity documents to upload

• Your child’s original birth certificate. If you are registering more than one child upload each child’s birth certificate
• Your government (US or foreign) issued identification such as a driver’s license or passport
  • Note: If you used your driver’s license as part of the residency supporting documents, you don’t need to upload it again

After you have uploaded your Proof of Identity, go to Step 6: Additional Documents
