Application for Homebound Services

Families or guardian(s) are requested to return the following to their school for completion of the application process:

1. Application: (English or Solicitud Para Educación Domiciliari—Español)
2. Medical Certification of Need
3. APS Release of Information

Please return the completed forms to:

• Elementary: return to the school principal
• Secondary: return to the school principal or director of counseling services

School teams are requested to submit these forms with the School Information Form on the Staff Homebound Google Site.
