As you file your annual tax returns below is some helpful information:

• The Employer Identification Number (EIN) for the Extended Day program is 54-6001128.
• To access your year end statement for tax purposes:
  1. Visit https://apsfamily.schoolcareworks.com/login.jsp to log in
  2. Click on the box that is named “Statements”
  3. Click on the drop down box where it says “view year” to choose the year then click on view to display the year that you are requesting
  4. At the top the right hand corner select “Summary Year End” and there you will be able to open the statement to view, save and or download the statement for tax purposes.
